I feel like the face is too flat? I'm not positive but it seems to be lacking some bone structure.

Since the piece is so busy (which I like) the only relief your eyes get is around the child's face, which is the focal point.
That said, since my eyes immediately go there I think the drawn eyes looked forced. It's as if everything is realistic except the eyes which feel too large and flat.
If this sort of manga style is what your going for I'd suggest emphasizing them even more. If it's not, making them smaller and have more curve would really help.
